Undergraduate Honor Code Incident Reporting Form


This form should be used to report potential Honor Code violations for undergraduates. Please provide detailed information regarding the incident you are referring to the Undergraduate Honor Council for adjudication. For more information about how to report graduate/professional cases, please review this website

If you are a member of the Vanderbilt faculty and you would like to learn more the Honor Council process before reporting, please do not complete this form. You may schedule a consultation with the Director of Student Accountability to discuss your options, which could include issuing a personal warning. Should you choose to issue a warning, a sample template warning is provided here for your customization and record keeping. More information on the Honor System can also be found in the Faculty Guide to the Honor System.

Do not use this form to report other violations of University policy, a student of concern, or acts of hazing that may violate University policy and/or Tennessee state law. For other violations of University policy, please make a report using the Incident Reporting Form. For a student of concern, please make a report using the Student of Concern Reporting Form. For acts of hazing that may violate University policy and/or Tennessee state law, please make a report using the Hazing Incident Reporting Form. Statement on Artificial Intelligence

At present, any allegation involving the use of artificial intelligence should be in alignment with the artificial intelligence guidance shared by the Center for Teaching and the Faculty Policy on AI provided by the Office of Faculty Affairs and Professional Education.

The allegation should be supported by factors indicating potential unauthorized aid (including, but not limited to, content inconsistent with assignment instructions, fake/dead-end links, inconsistencies between the voice and writing style of prior submissions, etc.) and not based solely on an artificial intelligence detector score (e.g., TurnItIn, zeroGPT, etc.).
