Complete this form only if you wish to appeal the decision of an instructor who has found you responsible for an academic integrity violation under WAC 504-26-202(1). If your appeal is submitted within 21 days of the instructor's written decision, a hearing will be scheduled with the Academic Integrity Hearing Board and will include you and the faculty member who submitted the initial report.
Students have the right to appeal an instructors decision in accourdance with WAC 504-26-415(d). This appeal process serves to ensure students receive an impartial review of decisions made by faculty in academic integrity incidents. The Academic Integrity Hearing Board will make a separate and independent determination of responsiblity for the incident appealed.
It is the responsibility of the student to submit evidence and present their appeal to the Academic Integrity Hearing Board. Once an appeal is submitted by a student, an Academic Integrity Hearing Board will be scheduled as soon as possible. The date, time and location of the hearing will be communicated to the student via thier WSU email.
An appeal is a review of a previous decision, in this case, the decision of the faculty member. The Academic Integrity Hearing Board can only make determinations in relation to WAC 504-26-415(c).
