If you have violated the Alcohol or Drug policy in the Student Code of Conduct, a letter informing them of this violation may be sent unless you show proof of your independent status by the date indicated on your outcome letter. Proof of independent status is defined as verification that you were not declared as a dependent on your parents' Internal Revenue Service Income Tax Return for the previous year. Such proof must be submitted as an attachment on this form.
