Sexual Misconduct Reporting Form (Title IX)

This form is to be used to report concerns or details of possible violations to the Wheeling University’s Sexual Misconduct Policy. Violations such as dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct are included. It is the policy of the University that designated staff members take immediate and appropriate steps to investigate or otherwise determine what happened when the University knows or reasonably should know of possible sexual misconduct. When sexual misconduct is determined to have occurred, the University shall take prompt and effective steps reasonably calculated to end the sexual misconduct, eliminate the hostile environment, prevent its recurrence, and, as appropriate, remedy its effects. All complaints shall be processed in accordance with the procedures established in this policy. In doing so, the University respects the sensitivity of the information included in this report and as mandated by Title IX as a federal law, the information included in this report is NOT confidential, but will be kept as private as possible in all efforts to protect the parties involved and the WU community.

To file a report with campus security, please call 304-243-2486 or go to the office located in the ground floor level of Swint Hall. Campus Security can assist in contacting local law enforcement if desired.
