Below is a form that may be utilized by any member of the College community to share concerns regarding the health, safety and wellbeing of a student.
Examples for referrals include:
Mental Health Support: individuals experiencing distress, concerning behavior or posing a threat to themselves or others.
Basic Needs Support: individuals experiencing significant financial obstacles such as: housing insecurity/homelessness, food insecurity, lack of transportation, lack of child care, medical injuries or hosptialization, and/or an urgent financial emergency that interfers with attending school.
A CARE team member may also be reached by calling 928-776- 2273 during regular business hours (M-F/8am-5pm).
This form should not be used to report emgergencies. For emergencies that require immediate attention, please call 911 or contact Campus Safety at 928-237-0420. Please then submit this form as a means of notifying the CARE Team of the situation.
While this report may be anonymous, it may limit our ability to respond. Providing your name and contact information allows a member of the CARE Team to contact you and learn more about your concerns.
