INCIDENT REPORTING FORM


Raritan Valley Community College urges all members of the college community to promptly report any incidents of student misconduct so they can be thoroughly investigated in a timely fashion. Please utilize this online form to report incidents of alleged violations of the Code of Student Conduct. If the incident relates to an issue involving an employee of the college or a visitor, please contact Human Resources or Campus Security. Although anonymous reports are allowed, they may limit the range of action that can be taken due to the inability to clarify specifics of the report. Including your contact information will allow the office to follow up if needed.

Reporting Methods:

1). Submit a report online using the form below.
2). Report an incident in person by visiting the Office of the Dean of Student Affairs within College Center Suite C-153. Normal business hours are (Monday – Friday, 8:30am – 5:00pm)
3). If immediate attention is needed, always contact Campus Security by picking up a red phone in the halls or calling (908) 231-8800. Do not use this form if you suspect a student represents a threat to themselves or others.
